FROM THE NORTH

From I-15 southbound, turn left (east) at the 600 North exit. Turn right (south) on 300 West. Turn left (east) on North Temple. Follow North Temple until it joins 2nd Avenue. Turn left (north) on B Street. Turn right (east) on 9th Avenue. Turn right (south) into the parking lot. Our office is on the corner of D Street and 9th Avenue in a tan brick building labeled 2 Medical Plaza.

FROM THE SOUTH

From I-15 northbound, take the 600 South exit. Turn left (north) on State Street. Turn right (east) on South Temple. Turn left (north) on E Street. Turn left (west) on 9th Avenue. Turn left (south) into the parking lot. Our office is on the corner of D Street and 9th Avenue in a tan brick building labeled 2 Medical Plaza.

Two patient parking lots are available for our patients.  For parking on the main level enter the lot from 9th Avenue, on the north side of the building.  An additional lot is available for patients on the lower level. This lot has direct access to the Infusion Center.  To access the lot take the upper ramp from the west side of the building on D Street.  The gates will rise automatically.  Both of these lots have wheelchair accessible entrances.  Parking is also available on the street for 2 hours.
